How to Torture a Pharmacist

Get him or her to optimize your medication regimen when you have multiple diseases, one of them is a seizure disorder, and you absolutely need to take phenytoin.

This just in: Phenytoin sucks.

I don't mean in the sense that it is a bad drug. It is quite effective. It's just really, really, really, really annoying.

It's that rare case of zero-order metabolism that makes it such a dosing nightmare. But then again, be glad you're not the patient, right?

And zero-order kinetics are just the beginning. That hepatic enzyme induction results in it not just inducing its own metabolism, but increasing the metabolism of a whole lot of other drugs.

But yes, you're quite right. I'd much rather dose phenytoin than be the patient. Seizure disorders are generally no fun.
